What does this PR do?

Retain valid remote settings for the active credential after fetch failures. Block providers when no safe config is available while keeping login usable. Preserve unavailable TUI model selections so Enter cannot silently switch models.

How did you verify your code works?

bun run check; focused core config/provider, server auth recovery, and TUI selection tests. Reproduced the failures before the fix using synthetic local services.

The following comment was made by an LLM, it may be inaccurate:

Cache the last successfully loaded wellknown manifest and resolved config per
origin in KV. A failed manifest or remote config fetch, or a missing credential,
falls back to that cache instead of dropping the remote providers and allowlist,
including on a cold start. Removing the source clears both.
